You have the right to:

  • File a consumer complaint

Requesting Health Care Records

You may request copies of your health care records by contacting me in any of the following ways:

Requests will be processed in accordance with applicable federal and state privacy laws (including HIPAA). You will be required to complete a Release of Information form.

Licensing Authority: I am licensed in Nevada and practice into Texas under PSYPACT authority. All licensing and disciplinary matters are handled by the Nevada State Board of Psychological Examiners (https://psyexam.nv.gov).

Filing a Complaint: If you have a concern regarding services, you may file a complaint with the Nevada State Board of Psychological Examiners using the link above.
